Clinical Pharmacists duties include provision of drug information, continuous quality improvement activities, development of policies and evidence-based practice guidelines, medication use evaluation, evaluation of new drugs for formulary, ongoing staff education and participation on Committees. Works with staff pharmacists to increase their participation in drug monitoring, pharmacokinetics and cost containment activities. Self-directed and able to prioritize multiple tasks to meet necessary deadlines. Able to perform the duties of staff pharmacist and supervisor as needed. Qualifications Education: Requires a bachelor or Pharm D. degree from an accredited school of Pharmacy. American Society of Health System Pharmacist Pharmacy Practice Residency preferred Licensure/Certification: Registered pharmacist with current Oklahoma licensure. BLS certification preferred. Experience: Minimum two years hospital pharmacy experience or American Society of Health System Pharmacist Pharmacy Practice Residency, specifically an ASHP accredited postgraduate year 2 (PGY2) residency in area of specialty preferred. Working Knowledge: Able to interpret, assess, compound and dispense all pharmaceuticals. Knowledge of appropriate drug information sources and general knowledge of word and data processing operations sufficient to perform data entry and generate appropriate reports Every team member at OHH plays an integral role in our patients' experience.
